This page will tell you what to expect when you arrive at our office on your first
visit and follow-up visits. For those of you who are considering care at our office please know that our policy is to offer
a FREE Chiropractic Consultation at the beginning
of your first visit. This will help us determine if you have a problem that we can help. The doctors combine their experience
and commitment to your health to determine the cause of your problems and the best
courses of treatment to correct them.
Most importantly, should you decide to come in for your free consultation, you can fill out
the new patient intake forms that we have provided and send them to us before you arrive,
reducing your waiting time in the office! We are easy to find, located just off Highway 101 in San Rafael and we have included directions to
our office for your convenience.

Schillinger Chiropractic & Wellness Center is a multidisciplinary office
providing you a variety of natural health care methods
to help improve your health without dangerous drugs or
needless surgeries. Our services include chiropractic
manipulation, massage therapy, acupuncture, physiotherapy,
weekly health-care classes, nutritional counseling, exercise
rehabilitation and lifestyle consulting. Providing
all these services under one health-care roof is practical
and beneficial to our patients. All of our staff members
meet weekly to review the quality of service and achievement
of results that we intend for our patients.
Knowing that our patients lead busy lives, we are open six days a week and are conveniently located less than 30 seconds
off of Highway 101. We have a large parking lot that is well lit and provides wheelchair access. When you arrive inside
our office, you will be greeted by a caring staff. You will be able to make yourself comfortable in a reception room that
looks and feels more like a living room. Most of the time patients can be treated immediately upon arrival and when there
are delays they rarely exceed 10 minutes.
Free Consultation -- Dr. Schillinger will spend approximately 15 minutes with you asking you questions
about your symptoms and, equally importantly, questions about your general health and lifestyle. If Dr. Schillinger determines
that you have a condition that he and/or our other health care practitioners can help, he will recommend an examination
involving chiropractic, orthopedic and neurological tests. He will explain this to you before examining you. Should you
have any questions about the fees for the examination, the Dr. Schillinger will be happy to review that with you before
the examination.
My Commitment Is to Your Health -- Dr. Schillinger
has 20 years of experience helping patients
improve their health. Dr. Schillinger gets to know each
patient and thier health care needs. If Dr. Schillinger
cannot help you or can only help you up to a certain
limit, he will refer you to the next best health care
practitioner.
In this era of managed care and decreasing loving care, Dr. Schillinger
will always be available to guide you in your health
care choices, whether you are an active patient in the
office or not. He is always willing to communicate with
your other health care practitioners whether it be by
phone or exchanging records whenever medically necessary.

What To Expect When You Arrive At Our Office.
First Visit:
Dr. Schillinger will spend approximately 30 - 45 minutes
consulting and examining you to determine if you
have a problem he can help. He will focus his attention
on your achievement of superior health in three key
areas; Spine and Structure, Nutrition and Psychology.
After the examination, if chiropractic care is appropriate,
treatment may begin (additional 15 - 30 minutes).
Second Visit:
Dr. Schillinger will spend approximately 15 minutes reviewing
the consultation and examination and x-rays (if they were needed) before commencing
a treatment plan specifically designed to correct your problem. If chiropractic
care is appropriate, additional 15 - 30 minutes is needed.
Third Visit:
Dr. Schillinger will spend approximately 5 -10 minutes
reviewing your treatment plan and answer any questions you have. If chiropractic
care is appropriate, additional 15 - 30 minutes. Questions you have regarding
your insurance benefits are answered, forms that you need assistance with
are filled out and all financial arrangements are completed.
Subsequent Visits:
Care is directed towards correction of your problem using the appropriate modalities in the
office and home exercise programs are initiated. You will be expected to attend the free workshop "Foundations
for Vital Living", where we discuss the benefits of an optimum diet, how to reduce mental/emotional
stress, how to safely (ergonomically) interact with your workplace and, of course, the importance of how and why to take
care of your spine, the source of all life-force in your body.
Re-Examinations:
Monitoring of your improvement is performed routinely to ensure that proper progress is being
accomplished. As you continue to improve, the frequency of your care diminishes...
Prevention Care:
Once your problem is corrected, recommendations are made for occasional checkups to prevent
a reoccurrence of problems or symptoms.
Wellness Care:
At this level of care, patients are symptom-free, sleeping better and able to adapt to physical,
emotional and chemical stressors. Patients come in for a chiropractic treatment as they simply want to experience a natural
increase in energy and vitality!

New Patient Intake Forms: Below we have provided the forms that we use in the office for
New Patient Intake, Workman's Compensation and Motor Vehicle Accident cases. Please download the appropriate forms, print
them out and fill them in prior to you visit. This will save you time on your first visit. Be sure to put the completed
form somewhere where will you not forget to bring it with you when you come to the office for the first time. If you have
any questions, you can contact our office and we will be happy to offer any assistance we can.
a) New Patient Intake Form for General Insurance, Private
Paying and Medicare Patients: please provide as much detail as possible
especially with regards to other chiropractic or medical physicians you have seen for this condition and the medications
you may be taking.
b) New Patient Intake Form for Workers Compensation Patients:
please provide as much detail as possible especially with regards to the exact matter in which you hurt yourself or, if
your injury is not arising from a specific injury, how your symptoms have developed from the repetitive nature of your
work.
c) New Patient Intake Form for Motor Vehicle Accident Patients: please
provide as much detail as possible especially with regards to the mechanics of the injury i.e. how fast the vehicles were
moving, what position your body was in at the time of impact and other care you may have received for this specific injury.
